Application for Child Care

Thank you for your interest in Hawfields! Please complete the following application to add your child to our waiting list. 

The enrollment process requires a completed initial application. As placement becomes available and parents are notified of time and classroom placement, a parent/child visit is scheduled. Introduction into the classroom is part of this visit. Questions regarding schedule and center operation are answered, for Parent and Director. 

Before your child's first day, the following forms must also be completed and returned:

  • Physical and Immunization Record (Immunizations are expected to be current upon enrollment. If the child’s immunizations are not up to date, the parent must agree in writing to a schedule for completing all immunizations in a timely fashion.)
  • Child Food Program Eligibility Application and Participant Enrollment Form
  • Permission Slip and Parent Agreement
  • Parent Authorizations

The registration fee and first week's tuition must also be received prior to the child's first day. The registration fee is non refundable. (See fee schedule for current fees and tuition).

Emergency Release
Hawfields Child Development Center agrees to provide transportation to an appropriate medical resource in the event of emergency. In an emergency situation, other children in the facility will be supervised by a responsible adult. I will not administer any drug or any medication without specific instructions from the physician or the child's parent, guardian, or full-time custodian. Provisions will be made for adequate and appropriate rest and outdoor play.
